Application Forms for Admission

You can download and print the applications below, complete all the applicable fields, and then bring it along with the required documents to the school office for registration.

Required upon Registration

The the following must be provided with the application package upon registration:
 

·   a recent photo of the applicant

·   a copy of the applicant’s passport or birth certificate

·   copies of the applicant’s three most recent report cards

·   a copy of the most recent transcript (if applying for Grades 10 to 12)

·   a copy of the applicant’s record of immunization
·  $100 (non-refundable) application fee
